Unless your school requires that you sign a release they provide, you may complete this form to request that your child’s records be sent to IMS. Upon submitting the form, an email complete with the information you provided and instructions for the sending organization will be emailed to the organization contact you list below. The sending organization may provide the records via mail, email, FAX, or file upload. We will email you when the records have been received.
