Request Student Records

IMS uses student records to further understand their experience in a school or care setting.

Unless your school requires that you sign a release, please complete the Student Records Request form to have your child’s records sent directly to IMS. Your child’s school/daycare will receive an email with the information you provided and instructions for how to send IMS the records via mail, email, fax, or file upload. We will email you when the records have been received.

Please complete the Student Records Request form at the same time that you submit your application to ensure that we receive the completed referral on time. If you have any questions about this step, please contact the office at or (919) 401-4343 for assistance.
