Teacher/Caregiver Referral

IMS uses a teacher/caregiver referral to provide a more complete picture of your child’s development. The teacher/caregiver you designate in the referral request form below will receive an email from IMS the next business day with a link and password to an online form. Please let the person know they will receive this form directly from IMS and ask that they complete it within five business days.

Please request a teachers/caregiver referral at the same time that you submit your application. If you have any questions about this step, please contact the office at office@imsnc.org or (919) 401-4343.
