IMS uses student records to better understand their experience in a school or care setting.
Please complete the Student Records Request form below to have your child’s records sent directly to IMS. Your child’s school/daycare will receive an email with the information you provided and instructions for how to send IMS the records via mail, email, fax, or file upload. We will email you when the records have been received.
